APPLICATION FORM FOR PHYSICALLY CHALLENGED SCHOLARSHIP AWARD
INSTRUCTION SHEET
1. GENERAL INFORMATION:
(a) Each candidate must complete this form fully and correctly directly,
together with the necessary documents.
(b) Application form submitted after the closing date, or
incorrectly/incompletely filled, or without the correct
documents will not be processed.
(c) Necessary documents (SECTION 3) should be sent as an attachment to
scholarship@ondostategovernment.com with your form autogenerated number as
the subject.
2. METHOD OF APPLICATION:
(a) Application forms are obtained by payment of prescribed fees to
designated Banks and on presentation of
Government Treasury Receipts to the Board.
(b) Students who are qualified to apply according to the above criteria should
collect application forms from
the board’s Office and submit the duly completed forms at the same venue.
(c) Candidates are advised to make only one entry. Those who make multiple
entries will have their entries
REJECTED.
(d) Completed application forms must be legible {Candidate are therefore advised
to write legibly}.
(e) Incomplete and or incorrect completion of forms will lead to AUTOMATIC
disqualification
3. ALL COMPLETED APPLICATION FORMS MUST BE ACCOMPANIED WITH PHOTOCOPIES OF
THE
FOLLOWING (USING THE FORMAT IN PARAGRAPH 6):
(a) Educational Certificates {on no account should original certificate be
sent},
(b) Letter of admission {issued by JAMB or Institution of Study},
(c) Course Registration,
(d) Last Sessional Examination Result,
(e) Identity Card,
(f) Letter of Identification from your Local Government Chairman,
(g) 2 recent full length size photograph showing the disability, with name and
form number at the reverse
side and
(h) Medical report from a Government Hospital confirming disability e.g. Visual,
Hearing etc.
4. SUBMISSION OF APPLICATION FORMS:
Applicants are to attach the SCANNED copies of
the certified true copy of 3(a)
to (h) and forward via email to
scholarship@ondostategovernment.com with your form's auto-generated
number in the subject line.
Please note that it is an offence punishable by
imprisonment to submit fake documents as the State Scholarship Board, State
Information Technology Development Center and EFCC have capacity to detect
fraudulent documents.
